REQUIREMENTS FOR PROVISIONAL REGISTRATION
The Applicant ( Completed M.B.B.S. from Rajasthan) is required to submit the following documents
: 1. Original Character Certificate with attested photostat copy thereof 2. Self addressed envelop size 9"x4" with postage stamps of Rs.37.00 for registered post.
3. D.D. Of
Rs.400.00
in favor of Registrar, Rajasthan Medical Council, Jaipur.
1. Provisional Registration Certificate of concerned State. 2. N.O.C. of Provisional Registration Certificate of concerned State.
3.
Character Certificate of concerned College 4. Passing Certificate of the concerned University 5. Date of Birth Certificate / Xth or XIIth Marksheet. 6. N.O.C. of the concerned College. 7. Internship starting order from Rajasthan Medical College .
FOR FOREIGN STUDENT 1. Original academic qualification documents from 10th to till date with it's photocopies.
REQUIREMENTS FOR PERMANENT REGISTRATION
The Applicant is required to submit the following documents in original with its
photocopies:
1. R.M.C. Original
Provisional Registration Certificate. 2. Original degree/diploma with attested photostat copy thereof.
3. Original
internship completion certificate issued by the College , with attested
photostat copy thereof. 4. Provisional certificate from the University(Original) in lieu of degree,with photostat copy thereof. 5. Affidavit to produce degree,when awarded.(as per performa on page 4 of application form) 6. Self addressed envelop size 12"x15" with postage stamps of Rs.42.00 for registered post.

Registrar, Rajasthan Medical Council, Jaipur.
REQUIREMENTS FOR ADDITIONAL REGISTRATION
Applicant is required to submit the following documents : 1. Original degree/diploma with attested photostat copy thereof. 2. Provisional certificate from the University(Original) in lieu of degree,with photostat copy thereof. 3. Affidavit to produce degree,when awarded ( as per performa on page 4 of application form) 4. Self addressed envelop size 12'x15 with postage stamps of Rs.42 for registered post. 5. D.D. of Rs 1000.00 per P.G. Qualification in favor of Registrar, Rajasthan Medical
Council, Jaipur.
REQUIREMENTS FOR OBTAINING DUPLICATE PERMANENT REGISTRATION CERTIFICATE
The Applicant is required to submit the following documents in original with its
2 photocopies: 1. Photocopy of Permanent Registration certificate issued by R.M.C. 2. D.D. of Rs. 500 in favour of Registrar, Rajasthan Medical Council, Jaipur. 3. Affidavit on Rs. 10/- non-Judicial Stamp Paper duely attested by Notary Public (Enclosed). 4. Self Address Envelop size 12’X15’ with postage stamps of Rs. 37.00 for registered post. 5. Copy of F.I.R.

7. Two copies of passport size photograph one affixed on the Form & one spare without attested. REQUIREMENTS FOR OBTAINING DUPLICATE ADDITIONAL QUALIFICATION REGISTRATION CERTIFICATE The Applicant is required to submit the following documents in original with its 2 photocopies: 1. Photocopy of Additional Registration certificate issued by R.M.C. 2. D.D. of Rs. 500 in favour of Registrar, Rajasthan Medical Council, Jaipur. 3. Affidavit on Rs. 10/- non-Judicial Stamp Paper duely attested by Notary Public (Enclosed). 4. Self Address Envelop size 12’X15’ with postage stamps of Rs. 37.00 for registered post. 5. Copy of F.I.R.

7. Two copies of passport size photograph one affixed on the Form & one spare without attested. REQUIREMENTS FOR OBTAINING GOOD STANDING CERTIFICATE
The Applicant is Required to submit the following documents in original with its
attested photocopies: 1. The application form should be properly and neatly filled in and sent in duplicate. 2. D.D. of Rs. 2000.00 only in favor of Secretary , Medical Council of India New Delhi, should be sent along with the application as fee. Cheques are not accepted. 3. Two testimonials of character and conduct as started in column no. “7” of this application form should be from persons of standing i.e. gazette Officers / Members of Parliament/Members of Legislative Assembly/Magistrate 1st. Class and the Principals and Professors of Medical College or from other persons of similar status and enclosed with the application in duplicate. 4. The names of the referees may be mentioned with complete and correct addresses to whom a reference could be made. These should not be the same persons who have issued a certificate as asked for in column no. “7” of this application form. 5. The application is to be forwarded to this office through the Register of the State Medical Council with whom the person concerned is registered. In case, he is registered with more than one State Medical Council, he should give all the registration numbers, with dates and the names of the States Medical Councils but forward his application through the Registrar of one of the Medical Council. 6. Copy of Registration Certificate should be enclosed. 7. D.D. of Rs. 500.00 in favor of Registrar Rajasthan Medical Council, Jaipur. 8. One envelop size 11” X 5” address to Secretary , Medical Council Of India, Pocket-14,Sector-8, Dwarka, New Delhi with postage stamps of Rs. 37.00 for registered post. 9. On character certificate please mention clearly name and seal of attesting Officer. Note: Sending Good Standing Certificate out side India a D.D of $ 100.00 only in favour of Secretary ,medical Council of India New Delhi should be sent along with the application as fee.Cheques are not accepted.
